Creating an Incident Report

This module is for NON-REFEREE submission of INCIDENTS that have taken place during
an event such as such as safety hazards, spectator behavior, volunteer behavior, or injury.

If you are a REFEREE, and witnessed an incident of any kind, please do not use this module.
We ask that you use the Incident Reporting functionality that you have access to in the Referee Tools.

Please expect to be contacted by some member of our AYSO staff as a followup to your report.
For the welfare of our players and volunteers, we take all INCIDENTS very seriously.
